We are the  first organization to achieve the INTERACT Center of Excellence designation. INTERACT (Interventions to Reduce Acute Care Transfers) is a quality improvement program, first designed in a project supported by the Centers for Medicare and Medicaid Services. This certification shows our commitment to providing the highest level of quality and positions us as a leader in reducing unnecessary hospital readmissions.

We have committed to a rigorous certification program that utilizes the INTERACT Quality Improvement Program (QIP). This includes interventions designed to improve the identification, evaluation and communication of changes in resident status.

Our participation in INTERACT includes an intensive focus on the following:

• Delivering leadership and direction for embracing full INTERACT QIP implementation
• Identifying best-practice approaches for utilization of the INTERACT QIP
• Monitoring data and clinical outcomes to identify areas for performance improvement
• Providing ongoing training and support for our care delivery team
